HOPE(Healthy living via Online suPport and Education) is an e-counselling programme that is designed to help you achieve your priorities for living well by making choices that improve your health.
As we go about our daily lives, we frequently face conflicting demands from our work, our families or our friends. Often, we have little time for ourselves and even though we know that our health is important, there seem to be no opportunities to do any more than just survive. However, life is not just about surviving, it is also about living well. We just do not seem to be able to find the time or the energy to change the way we live our lives. Even when we do find time, we do not know what to do or feel confident that we can make any meaningful changes.
To support you in your journey to live well, a group of experts including physician, psychologist, physiotherapist, nutritionist, pharmacist and smoking cessation programme manager came together to create HOPE. We will work with you to develop your personalised programme by identifying your priorities for living well, selecting self- care behaviour(s) that match your priorities and building your motivation and confidence to initiate and maintain positive changes in your life. Each session will bring you closer to your priorities of living well and a healthier life.By following through the HOPE programme, you will be able to:
identify and develop your personalised self-care plan,
identify barriers that may interrupt your self-care plan,
develop coping skills to overcome barriers so you can stay on track with your plan,
achieve your priorities for living well and take charge of your health.
